The entries are saved in a plain text file, for easy editing, and there is a naughty word filter as well. Have a look at the Default Guestbook Example, and the Voidspace Guestbook which makes use of the HTML templates to integrate with Voidspace. Almost every aspect of the appearance can be edited using the templates. Various user settings can also be configured. Full instructions are contained in the template file. It can email you when an entry is made, and features smilies, anti-spam protection, javascript form validation, and allows a limited set of html in comments. It is regularly improved and updated.
